The Red Bank Office of Vital Statistics maintains records of Births, Deaths, Marriages, Civil Unions and Domestic Partnerships for events that occur in Red Bank only.  Copies may be obtained in person during regular business hours, by mail or ordered through the Vital Check website.  Information on how to order can be found below.

Applications for Marriages/Civil Unions for Red Bank residents are done by appointment only.  Please call (732)530-2755 to make an appointment and to find out what is required at the time of application.  Please be advised, there is a 72 hour waiting period between the time of application and the time the license can be issued. 

Applications for Domestic Partnerships (available only to individuals 62 years and older) for any New Jersey resident are also done by appointment.  Please call (732)530-2755 to make an appointment and to find out what is required at the time of application. 

Ordering a Vital Record

All applicants must show a valid photo driver's license with their current address.  For those that do not have a valid photo driver's license, two alternate forms of identification are required, one of which must show the applicant's current address.  A list of acceptable ID's can be seen on the State of New Jersey Department of Health and Senior Services website

Note:  Anyone whose name has changed, such as women who are using a married name and trying to obtain their birth record or women who have remarried, are using a new name and trying to obtain a record for their child under the previous name, will also need to provide a certified copy of their marriage license showing the change to the new name.

To order by mail applicants must submit an Application for a Certified Copy of a Vital Record or a written request providing the required information as noted.


Birth Records

Birth records are available to the subject of the record, either of the subject's parents, spouse, adult siblings, adult children and adult grandchildren as well as their legal guardian or legal representative.  All relationships must be proven with certified copies of the appropriate record.  Requests for birth records must include: 

  • Full name of child on the record
  • Exact Date of Birth
  • Full name of Father as it appears on the record
  • Full name of the Mother as it appears on the record
  • Number of copies requested
  • $10 for each copy ordered
  • Copies of acceptable identification

Marriage/Civil Union Records

Full copies ofMarriage/Civil Union Records are available to the Bride and Groom or partners only.  Limited copies are available to the subjects' parents, spouse, adult siblings, adult children and adult grandchildren as well as their legal guardian or legal representative.   All relationships must be proven with certified copies of the appropriate record.  Requests for marriage/civil union records must include:

  • Full name of Groom/Partner A
  • Full maiden name of Bride/Partner B
  • Exact Date of Ceremony
  • Location of Ceremony
  • Number of copies requested
  • $10 for each copy ordered
  • Copies of acceptable identification

Death Records

Death records are available to the surviving spouse of the subject of the record, either of the subject's parents, adult siblings, adult children and adult grandchildren as well as their legal guardian or legal representative.  All relationships must be proven with certified copies of the appropriate record.  Requests for death records must include: 

  • Full name of Decedent
  • Exact Date of Death
  • Full name of Decedent's Father
  • Full maiden name of Decedent's Mother
  • Number of copies requested
  • $10 for each copy ordered
  • Copies of acceptable identification

Domestic Partnership Records

Domestic Partnership Records are available to the partners on the record, the subjects' parents, adult siblings, adult children and adult grandchildren as well as their legal guardian or legal representative.   All relationships must be proven with certified copies of the appropriate record.  Requests for domestic partnership records must include:

  • Full name of Partner A
  • Full name of Partner B
  • Exact Date Partnership was registered
  • Number of copies requested
  • $10 for each copy ordered
  • Copies of acceptable identification
