Ordering a Vital Record

Ordering In Person

All applicants must show a valid photo driver's license with their current address. For those that do not have a valid photo driver's license, two alternate forms of identification are required, one of which must show the applicant's current address. 

Note

- Anyone whose name has changed, such as women who are using a married name and trying to obtain their birth record or women who have remarried, are using a new name and trying to obtain a record for their child under the previous name, will also need to provide a certified copy of their marriage license showing the change to the new name.  

- Requestor ordering a copy of a relative's vital record, must prove relationship, 

Example:

1. for a sibling's record: requestor must provide a birth certificate showing common parents.

2. for death certificates: executor papers or a birth certificate showing common parents.

In addition, any name change must be proven as well. 

Ordering by Mail

To order by mail applicants must submit an Application for a Certified Copy of a Vital Record (PDF) or a written request providing the required information as noted.
